They have such a cute sweet relationship. There are so many "older guy and small child" relationships (no, not the ones that are nasty!) these days. But this is the best (and probably origional) one.

He was in an accident when he was about 7 - 9 Then its been 20 years sence the accident soo...he's about 27 or 29.

Well in episode 42, thats the episode that Black Jack first meets Pinoko. Thats when her and twin sis were one and so, he did a operation and so he really is 18 but she is in the body of here younger siter

pinoko was a rare type of parasitic twin, living inside of black jack's patients for 18 years until he extracted her and gave her a body. After being rejected by her sister, she ended up living with black jack.

She was actually a twin growing inside her twin. This is called Teratogenous Cystoma, or teratoma. This caused Pinoko's twin to have a large tumor. BJ opened the tumor and in there was Pinoko as a pile o' guts. He gave her an artificial body and so we see Pinoko as today. This is impossible in real life though. The twin inside the other is usually dead.
